Clinic Fee and Registration
The advance registration fee for the Clinic is $95 per person and includes:

All attendees will receive a confirmation notice and invoice(if applicable) by e-mail once you register.  The fee for on-site registrants will be $110 per person.

Payment
Your clinic fees may be paid by one of the following methods:

Payment MUST be received within 5 days of registering on-line to be registered for the clinic.  If your payment is not received prior to December 12, 2008, you will be required to pay the on-site registration fee of $110 in order to attend the clinic.

Hotel Reservations
A block of rooms has been reserved at the Holiday Inn Select - Executive Center for the MTCCCA Clinic.  The block is being held until November 13, 2008, and the discounted room rate of $76.95 (plus taxes) per room per night is valid for 1 - 4 persons per room.  Make your reservations by calling the Holiday Inn Select directly at 573-445-8531 and asking for the MTCCCA group block.  Please call by November 13, 2008, to guarantee that you will receive the discounted rate.
