Clinic Fee and Registration
The advance registration fee for the Clinic is $100 per person and includes:

All attendees will receive a confirmation notice and invoice(if applicable) by e-mail once you register.  After December 1, 2009, the registration fee is $115.00. All attendees will receive a confirmation number and invoice (if applicable) by e-mail.  If you do not receive an e-mail confirmation, please contact Jamie Schieber (573) 445-2965 to confirm you are registered.

Payment
Your clinic fees may be paid by one of the following methods:

Hotel Reservations
A block of rooms has been reserved at the Holiday Inn Select - Executive Center for the MTCCCA Clinic.  The block is being held until November 23, 2009. The discounted room rate of $78.95 (plus taxes) per room per night is valid for 1 - 4 persons per room.  Make your reservations by calling the Holiday Inn Select directly at (573) 445-8531 and asking for the MTCCCA group block.  Please call by November 23, 2009, to guarantee that you will receive the discounted rate. Please remember to ask for the MTCCCA discounted rate.
